All ban/unban requests should be in the following format:

Name: (Your Name)
Rank: (Your Rank; N/A if not in the unit)
Date/Time of Incident: (The date, time and time zone)
Suspect: (Name of the person you are reporting, N/A for unban)
Steam ID: (Steam ID of the person you are reporting, Your's if unban)
Account of Incident(What Happened):
Witnesses(Must be an active fraG member):
Additional Comments:
